Weekend link: Blameless postmortens

Why You've Never Been In a Plane Crash is about aviation safety – but the approach to safety is applicable in many other areas as well.

It’s often much more productive to ask why than to ask who. In some industries, this is called a “blameless postmortem,” and in aviation, it’s a long-standing, internationally formalized tradition. [...] In a system where lives may depend on the accuracy of a single person, disaster is not only probable but, given enough time, inevitable.

What is more important: Find the human that made a mistake this time, or find the systemic problem that opened a door to human mistakes in the first place?
